Our Services

At SDCS we are committed to our mission of caring, empowering and serving the people in need of supports. Because every individual we support is different, we strive to provide services that are specific to each individual’s vision, needs and goals. Specialized Direct Care Services, specializes in services which are tailored for each individual.

1. Community Integrated Habilitation

The Community Integration and Habilitation (CIH) Waiver program provides Medicaid Home and Community-Based Services (HCBS) to participants residing in a range of community settings as an alternative to care in an intermediate care facility for individuals with intellectual disabilities (known as an ICF/IID) or related conditions. 

The CIH Waiver serves individuals with an intellectual/developmental disability or autism who have substantial functional limitations, as defined in 42 CFR 435.1010. 

To qualify for our services under the CIH Waiver, the individual must be approved by the  Division of Disability and Rehabilitative Services (DDRS). 

Under these programs, individuals may choose to live in their own home, family home, or community setting appropriate to their needs. SDCS will then provide staffing to serve these individuals as required by each individual’s Person Centered Support Plan (PCISP) .

GOALS and OBJECTIVES: 

The CIH Waiver programs provides access

  • to meaningful and necessary home and community-based services and supports. 

  • to services and supports in a manner that respects the individual’s personal beliefs and customs. 

  • to services that are cost-effective.

  •  to the individual’s involvement in the community where he/she lives and works.

  • to the individual’s development of social relationships in his/her home and work communities.

  • to the individual’s independent living settings.


 

SDCS provides the following services under the CIH Waiver:

  • Community Based Habilitation - Individual

  • Community Transition Services

  • Facility Based Habilitation - Individual

  • Family and Caregiver Training

  • Residential Habilitation and Support 

  • Respite 

2. Family Support Waiver

The Family Supports Waiver (FSW) program provides Medicaid Home and Community-Based Services (HCBS) to individuals residing in a range of community settings as an alternative to care in an intermediate care facility for individuals with intellectual disability (known as an ICF/IID) or related conditions. 

The FSW serves persons with an intellectual/developmental disability, intellectual disability or autism and who have substantial functional limitations, as defined in 42 CFR 435.1010. Individuals may choose to live in their own home, family home, or community setting appropriate to their needs.

Each individual’s team (IST) develops an Individualized Support Plan (ISP) using a person-centered planning process. The IST consists of the individual, the individual’s case manager and any other stake holder in the service delivery. 

The individual, with the IST, selects services, chooses service providers and develops a Plan of Care/Cost Comparison Budget (POC/CCB).

3. Structured Family Caregiving

The Family Supports Waiver (FSW) program provides Medicaid Home and Community-Based Services (HCBS) to individuals residing in a range of community settings as an alternative to care in an intermediate care facility for individuals with intellectual disability (known as an ICF/IID) or related conditions. 

The FSW serves persons with an intellectual/developmental disability, intellectual disability or autism and who have substantial functional limitations, as defined in 42 CFR 435.1010. Individuals may choose to live in their own home, family home, or community setting appropriate to their needs.
